For filed births that occurred in Northeast Ohio counties only; or for filed deaths that occurred in Portage County, Ohio.

 1. REQUEST IN PERSON PICK UP  ("Will Call"). Be sure to choose  "Kent City Department of Health" from the agency dropdown list on the "Select a Birth Certificate" page when ordering. When completing the required shipping information page- all fields marked with a red * are needed for billing & receipt purposes  - Click the green circled box below to place an order for pick up:

Print, review, complete and submit the Application for Certified Copies- Birth Record (PDF) or Application for Certified Copies- Death Record (PDF).

KCHD Vital Statistics Office is located on the 2nd Floor of the Kent Central Gateway/PARTA Bus Terminal Building- West Entrance Only. (Corner of E. Erie St. / S. Depeyster St.). 30 minute** parking is on the 4th level WEST SIDE of the parking garage. Enter from S. Depeyster St. (**15 minutes free) or use any available metered street parking.

• Certified copy orders can also be submitted in-person 8am until 3:30pm Monday-Friday or through the mail; please print, complete:
  Application for Certified Copies- Death Record (PDF).

• Burial permit issuance fee: $3.00
